The Prince Edward Family Health Team (PEFHT) is one of 200 Family Health Teams across Ontario, which opened as part of an Ontario Ministry of Health and Long Term Care initiative aimed at improving access to primary health care professionals in Ontario. Our family health team provides a single point of access to health care services for all County residents. Our interdisciplinary care teams offer comprehensive patient-centred, primary health care including disease diagnosis, treatment and management, prevention and cure, rehabilitation, palliative care and community health promotion.

Health Care Connect What is Health Care Connect? Health Care Connect refers Ontarians who don’t have a physician to a family health care provider who may be accepting new patients. How does Health Care Connect work? Unattached patients can call or go online to ontario.ca/healthcareconnect to register with the program. To successfully register, unattached patients must have a valid OHIP card and complete a health questionnaire to determine their need for family health care services. Priority is given to individuals who have greater health needs. Once registered, program participants are assigned to a Care Connector.

Mental Health Support Network is a Ministry of Health and Long-Term Care funded agency which provides support and services within to individuals who have been diagnosed with a serious mental illness or who have experienced difficulties with mental illness. The Picton centre is currently open every other Wednesday but plans are being made to expand this schedule.
